The periosteal attachment offers hindrance to flap advancement specially during grafting procedures and periosteal release incision (PRI)helps overcome that . With this release incision the flap can be easily advanced over the surgical site .This video demonstrates the PRI and the flap advancement after that . Also after the incision it is important that , the surgeon must assess the closure which must be tension free with the flap gently stretched across the surgical site . This can be appreciated towards the end of the video .
Piezosurgery utilizes piezoelectric vibrations for cutting bone tissue. By adjusting the ultra sonic frequency of the device, it is possible to cut hard tissue without damaging the soft tissue.

The placement of implants in edentulous areas is often compromised by a thin alveolar crest. This reduction in the width of the edentulous ridge may make the final prosthetic results less than ideal. To correct this, various techniques have been described for
widening, including revascularised block bone grafts, alveolar distraction
osteogenesis, guided bone regeneration, and splitting to
expand the ridge. However, these methods have limitations,
including the need to harvest bone from intraoral or
extr-oral sites along with increasing the time frame for the entire treatment during which the bone conditioning takes place which can be many months in a lot of cases before the implants can be placed.
With the Osteotome Technique displayed in this video the implant osteotomy can be prepared without any ostectomy / bone removal and the implants can be inserted in the prepared sites . However to use this technique there needs to be a minimum 3mm of pre-existing bone width at the desired implant placement site .

As is well known that MB2 Canals are found in more than 50-60%of maxillary 1st molars and most of the times they are not easily located . Failure to properly clean and obturate this canal usually leads to a failed endo treatment in many of these cases. A serious attempt needs to be made to locate the MB2 specially in re-treatment cases and by giving sufficient clinical time , use of magnification and ultrasonic instrumentation for micro-endodontics , which increases the success rate of finding the MB2. In this video I show locating one such MB2 during the re treatment of a Maxillary Molar . The root canal of this patient had failed previously due to a missed out MB2 and had been performed 15 yrs back.

This video is of an endodontic retreatment case of tooth 37 ( mandibular 2nd molar ) which had been previously treated around 5 yrs back. Re-entry led to spontaneous exudation of blood and pus from a perforation near the MB Canal . Localization of the perforation site was done under the microscope with copious irrigation and curettage of the granulation tissue. Even after extensive irrigation there was still slight amount of exudation from the perforation site, this exudation is visible towards the end of the video . The case is under followup and we'll be posting videos of the perforation repair as it proceeds.
